Community Awards Committee:  
Each nominator will be notified, indicating that their nomination has been received by the Committee. Individuals or groups who have been selected to receive Town of Gravenhurst recognition will be honoured at a special presentation ceremony to be held at the Gravenhurst Opera House on Wednesday, November 19 at 7 pm.

Describe nominee’s commitment to Volunteerism, Outstanding Achievement Beyond the Local Level or Act of Good Samaritan. Attach additional details that clearly illustrate the impact this person or group has had in our community. Example: Letters of support, newspaper articles, other media attention, reports and other forms of recognition should be included if possible. If the nominee is a group or team, please list all members of the team and include a primary contact person; including their phone numbers and addresses on a separate sheet of paper.
